Also, what account is ok for getting the cash back? Is it really cash back, or does one get a gift card for only certain places?

It's free, and they pay you with paypal. It took 60 days to get my my first Live cash payment, and a couple I'm still waiting on. It just wasn't worth it to wait 60 days, so I switched to www.PaymentKey.com which pays in 15 days.
But I love PaymentKey because it's serious cash. It works with hundreds of stores including eBay to the tune of about 11% but that varies, but not too much less and many times more.
